In the mirror universe, the Caretaker was the name given to the Nacene entity who assumed responsibility for the Ocampa.
By the 24th century, the reason why the Caretaker protected the Ocampa had been lost to history. One legend stated that, centuries earlier, he had driven off invaders who had destroyed the surface of Ocampa V and had then built the underground city to protect them against further invasion while another stated that he was preparing the Ocampa for a great task which he had chosen them to perform.
By 2371, the Caretaker had begun transporting numerous ships from the Alpha Quadrant across a distance of more than 70,000 light-years to Ocampa space for unknown reasons using an array. While attempting to escape from the Kazon-Ogla, the ship on which the Talaxian trader Neelix and his Ocampa lover Kes were travelling was caught in a displacement wave generated by the Array and was sent to the Badlands in the Alpha Quadrant. Once there, the ship was destroyed by one of the Badlands' characteristic plasma storms but not before Neelix was able to launch two escape pods. While he was rescued by the Terran Rebellion fighter Geronimo, Kes was captured by the Klingon-Cardassian Alliance warship Bak'rikan. (VOY - Mirror Universe novel: The Mirror-Scaled Serpent)
- While the reason that the Caretaker was transporting vessels to the Delta Quadrant was not stated in The Mirror-Scaled Serpent, it is reasonable to assume that, as was the case with his primary universe counterpart in VOY episode: "Caretaker", he was dying in 2371 and was consequently searching for a biologically compatible lifeform with whom he could reproduce. Furthermore, VOY episode: "Cold Fire" revealed that the Caretaker's species, the Nacene, visited Ocampa during the 14th century and accidentally devastated the surface of the planet, leading to the Caretaker assuming his protective role over its people. This was presumably also the case in the mirror universe.
