Kobayashi Maru.

The ECS Kobayashi Maru (or the SS Kobayashi Maru) was a neutronic fuel carrier freighter, lost near the Tezel-Oroko system, near the Federation-Klingon border in 2155. Her captain was Kojiro Vance. (TNG novel: A Time to Heal, ENT novel: Kobayashi Maru)

The loss of the Kobayashi Maru formed the basis of a Starfleet Academy test simulation for Starfleet Command School cadets, known as "The Kobayashi Maru Scenario". A test of how an individual handles a no-win situation, has the cadet in command of a Starfleet vessel, charged with responding to a distress call from a crippled Kobayashi Maru, which is then threatened by hostile forces. Background

The ENT novel: Kobayashi Maru touches on the details regarding the loss of this vessel.

In the novel, the ship is presented as an old, retrofitted Klingon vessel which is now under the control of the Earth Cargo Authority and crewed by humans, including Captain Kojiro Vance. Its 300 passengers, mentioned in TOS movie: Star Trek II: The Wrath of Khan, are revealed to be Vulcans who are tasked with establishing a secret listening outpost; the Kobayashi Maru is subsequently destroyed by attack vessels, also of Klingon design but under the remote control of the Romulans.
