Forma was a star system, located within the galaxy's Alpha or Beta Quadrant, in or near Federation space, near the Klingon Empire's border with the Romulan Neutral Zone.

The Forma system's orbit was the location of a number of worlds, including sixth planet Forma VI.
In the 22nd century, Human colonists established a dilithium mining outpost in the system.
In 2273, the IKS Kandar and USS Enterprise visited the system in hopes of locating replacement dilithium. Enough crystals were found for both ships, but Kandar's captain Kodrash suspected foul play and destroyed a Federation travel pod containing Klingon officers and his supply of dilithium.
