In the Kobayashi Maru scenario, the distress call sent out from the stricken SS Kobayashi Maru stated that it has been struck by a gravitic mine and disabled. (TOS movie & novelization: Star Trek II: The Wrath of Khan)
Gravitic mines were extensively used during the Federation-Cardassian War in the 2350s and 2360s. Following the war's conclusion, several of the mines were left in place and weren't disarmed. In 2368, the USS Denver fell victim to one of these mines while traversing the Beloti sector. (TNG episode: "Ethics")
Gravitic mines are also used by the Bajorans and the Cardassians. (DS9 video game: The Fallen)
In 2370, the Albino used an anti-personnel gravitic mine at the main threshold of his compound on Secarus IV. (DS9 episode: "Blood Oath")
The Dominion have been known to use gravitic mines. In 2375, a Federation shuttlecraft carrying Ezri Dax, Nog, Jake Sisko and Ensign Warren struck a cloaked gravitic mine near a planet they were investigating. The team later theorized that the mine had been placed there by the rogue Breen they encountered on the planet. (DS9 - Strange New Worlds V short story: "Fear, Itself")
