In the mirror universe, the ISS Voyager (ISS-74656-A, also NCC-74656-A) was a 24th century Imperial Terran starship, a Lamarr-class science vessel in Imperial Starfleet service in the 2380s decade. Its commanding officer was Kathryn Janeway. (PRD episode: "Cracked Mirror")

In the 2370s, Captain Janeway commanded the Intrepid-class starship ISS Voyager. Chakotay served as her first officer following the death of Aaron Cavit. (VOY comic: "Mirrors and Smoke")

Voyager carried the registry number ISS-74656-A on the hull. A master systems display on the bridge also showed NCC-74656-A.

In 2384, Commander Chakotay served under Admiral Janeway aboard the Voyager-A. In that year, Voyager-A captured Starfleet personnel from the primary universe - Captain Chakotay of the USS Protostar and cadets from the USS Voyager-A. Janeway and Chakotay interrogated their captives in the brig, threatening them with time in the agony booth. The ship came under attack from timeline-devouring aliens. Captain Chakotay and his wards escaped back to the primary universe. (PRD episode: "Cracked Mirror")

By the 2410s, Janeway served as the marshal of the Terran Empire and commanded an upgraded version of the original ISS Voyager during an assault on the Federation in the primary universe. (STO - Terran Gambit mission: "Red Shift")
