Lex Nakashima was a male Human in the 23rd century. By the late 2270s, Nakashima was serving in Starfleet as a sciences division lieutenant, assigned as a junior science officer aboard the USS Enterprise. (Star Trek II Short Stories)

Nakashima's first visit to an alien planet was in 2277, as part of a rescue party with Rear Admiral James T. Kirk on the surface of Varda III. While searching through an ancient stone structure, he worried about danger at each turn, then recalled Spock's advice that fear did not equate to panic, and was able to refocus his thoughts on the excitement of exploring the unknown. Nakashima was manning the science station on the Enterprise's bridge as the starship approached Pollox IV in 2277. After detecting a starship at extreme sensor range, he reported the finding to Admiral Kirk. (TOS - Star Trek II Short Stories short story: "The Blaze of Glory")

Later that year, Nakashima joined Doctor McCoy and Lieutenant Commander Uhura in the Enterprise's rec room when they were all off-duty. Nakashima was excited about the Enterprise's upcoming liberty on Macedon III and spoke at length about all of that planet's tourist attractions, leading McCoy to believe that he was working for the Macedon Chamber of Commerce. Nakashima was storing Hikaru Sulu's decontaminated insect samples from Marris V when he became infected by a phytoplankton bacteria. It produced a compulsion to spread the infection by touch, and he gave it first to Sulu. He and Sulu then attempted to infect Kirk in a corridor, but the captain had inadvertently been inoculated by a weight-loss injection. Later, Nakashima was among the crew who were incapacitated with Morpheus-Nine anesthesia gas and then cured with shots of a saline solution and vitamins. Nakashima was part of a rescue party that beamed down to Omicron Theta V. (TOS - Star Trek II Short Stories short story: "The Secret Empire")

As the Enterprise flew disabled through a highly energetic ion storm, a photon torpedo malfunctioned. Nakashima fired it into the storm, preventing any damage. (TOS - Star Trek II Short Stories short story: "To Wherever")

Nakashima accompanied Kirk, Dr. Christine Chapel, Sergeant Juan Aragonés, Barry St. John and Marta Bergstresser on a landing party to Charlotte, one of the three moons of the planet Cathy, where they explored a cave system containing one-third of the lost treasure of Vulcan.
