The President of the United States of America is both the head of state, and the head of government for the United States of America, a political union on the North American continent on Earth.
The office of president was established by the ratification of the Constitution of the United States of America in 1787. Prior to the ratification of the Constitution, under the Articles of Confederation the executive was known as the President of the United States in Congress Assembled, and over a dozen men served had served in that office. George Washington was the first person elected to the office of President under the Constitution. (ENT novel: The Good That Men Do)
In the early 20th century, giant likenesses of four of the most admired presidents were carved out of the granite in Mount Rushmore in South Dakota. A fifth likeness, that of Sarah Susan Eckert, was added sometime prior to 2287. (TOS novelization: Star Trek V: The Final Frontier)
By the early 23rd century, the office of President of the United States had become a largely ceremonial role, rather than a position with actual political power. Ambassador Sarek once gave a speech on computer systems to a number of dignitaries, including the president. (TOS novel: Spock's World)
In 2375, following the Breen attack on San Francisco, the president joined Federation President Min Zife and the Prime Minister of United Earth in touring the city. (ST short story: "Eleven Hours Out")
