Puerto Rico is an island in the Caribbean Sea on Earth. Originally settled by American aboriginals, it became a colony of Spain in the 16th century, then became a possession of the United States of America in 1898.

Sonya Gomez was a native of Puerto Rico, and spent her leave time following the near-destruction of the USS da Vinci at her parents' home in the city of Vieques. (SCE eBook: Breakdowns)

Puerto Rico's political status in the Star Trek era is unknown, other than the assumption it is part of United Earth. It may possibly have become a U.S. state (re: the 52-star flag seen in TNG episode: "The Royale"), or it may be part of the Caribbean Alliance (established in TOS novel: Vulcan's Heart).
