Theta IV-Z was a location where, in 2355, Starfleet sent a team to which included Zach Warner and Benjamin Sisko.
An incident occurred which botched the mission and Sisko placed the blame entirely on Warner. Warner's involvement was unknown to all except Sisko and the younger officer did not tell anyone of the fact, knowing it would end Warner's career.
