Theta IV-Z was a location where, in 2355, Starfleet sent a team to which included Zach Warner and Benjamin Sisko.

An incident occurred which botched the mission and Sisko placed the blame entirely on Warner. Warner's involvement was unknown to all except Sisko and the younger officer did not tell anyone of the fact, knowing it would end Warner's career. In 2374, Sisko contacted the now Vice Admiral Warner and called in a favor for not revealing the truth about what happened at Theta IV-Z. (DS9 - The Dominion War novelization: Call to Arms...)
