Memory Beta, non-canon Star Trek Wiki

A transporter duplicate was a lifeform created when a transporter accident created two copies of the same person.

In 2266, two incomplete duplicates of Captain James T. Kirk were created when he was beamed from Alfa 177. The duplicates were re-merged into one individual. (TOS episode: "The Enemy Within")

In 2361, a duplicate of Lieutenant William T. Riker was created when he was beamed from Nervala IV. This was not discovered until 2369, when the USS Enterprise-D visited the planet. (TNG episode: "Second Chances")

In 2381, a duplicate of Lieutenant junior grade Brad Boimler was created when he was beamed from Karzill IV. (LD episode: "Kayshon, His Eyes Open")

Later that year, Doctor Iona created multiple transporter duplicates of Captain Carol Freeman and placed them in holodeck scenarios designed to force her to reveal the prefix code for the USS Cerritos. When Freeman proved too intransigent, Iona began creating duplicates of Ensign Beckett Mariner instead, one of whom sacrificed herself to destroy Iona's ship and save the Cerritos. (LD comic: "Warp Your Own Way")
