The USS Sheridan (NCC-4463) was a Larson-class Federation starship in service in the 23rd century.
On the Sheridan's maiden flight she experienced a critical overload in her warp drive system and all back-up systems had failed to correct the problem. It was decided to eject the ship's engine pods because an uncontrolled matter-antimatter mix was underway and couldn't be stopped, but unfortunately, the ejection systems also failed to activate and the Sheridan was destroyed in the subsequent explosion.
Only three survivors were recovered as they had taken refuge inside a shuttlecraft when the crisis began. (FASA RPG module: Federation Ship Recognition Manual)
